Wedding music application

You may obtain a copy of the Wedding Music Selection Form by clicking HERE. (adobe pdf format) or contacting me at (305-534-3025) or (Fax: 305 538-1345). A copy of the form will be mailed, faxed or e-mailed to you. You should complete and return the form to the organist at least one month in advance of the wedding.
